Individual assignment

The transfer of Takaful benefits from certificate owner (Assignor) to another person or institution (Assignee). The Takaful benefit can be assigned partially or up to 100%.

Requirement:

If the assignee is individual:

  1. Individual assignment (PSF19) form with stamp duty
  2. Tax residency self-certification form (Individual) duly completed by the Assignee
  3. Both Assignor's and Assignee's clear copy of NRIC

If the assignee is company / organization:

  1. Individual assignment (PSF19) form
  2. Questionnaire for authorized person of the company
  3. Form 8 - Certificate of Incorporation for Public Company OR
  4. Form 9 - Certificate of Incorporation for Private Company
  5. Form 24 - Return of Allotment of Shares (Latest)
  6. Form 49 - Return giving particulars in register of directors, managers and secretaries and changes of particulars (Latest).
  7. Authorization letter from the company or resolution from Board of Directors for the appointed authorized person
  8. Clear copy of NRIC/passport of the authorized person
  9. Tax residency self-certification form (Entity) and Tax residency self­ certification form (Controlling person) - Pages 3 and 4

In the event of the death or Total and Permanent Disability of the ASSIGNOR the assigned benefits as stated in the form will be given to ASSIGNEE.
