Change of ownership

Juvenile certificate:
Person covered is a minor and the certificate owner who passed away or divorced can be replaced with a new certificate owner (biological parent) or court appointed legal guardian. In the event of the certificate has no contributor waiver (CBP), inclusion of the rider will be subjected to medical underwriting.

Spouse certificate:
Person covered can be new certificate owner where the spouse who is certificate owner but was passed away or divorced.

Keyman certificate:
The employer who is the initial certificate owner agreed to transfer the ownership to the person covered as the new certificate owner.

Requirement:

a) Statutory Declaration on Change of Ownership completed and signed by the existing and new certificate owner. The SD2 Form must be stamped with RM10 stamp duty and signed and witness in front of the Commissioner of Oath.

b) Declaration by new participant on change of ownership form

c) Clear copy of the new certificate owner’s NRIC/biometric copy or a clear copy of valid passport is needed.

d) Proof of relationship between the certificate owner and the new certificate owner. Example: Divorce certificate (Surat Perakuan Cerai).

e) Proof of relationship between the new certificate owner and the minor/person covered. Example: Birth certificate, court appointed legal guardian or legal adoption certificate (apply to juvenile certificate).

f) Tax residency self-certification form (Individual) of the new certificate owner.

g) Request for contractual changes form if the certificate has waiver rider for the existing certificate owner.

a) Declaration of new participant upon death of certificate owner form

b) Clear copy of the new certificate owner’s NRIC/biometric copy or a clear copy of valid passport is needed.

c) Death certificate of the deceased certificate owner.

d) Proof of relationship between the deceased and the new certificate owner. Example: Marriage certificate.

e) Tax residency self-certification form (Individual) of the new certificate owner.

f) Proof of relationship between the new certificate owner and the minor/person covered. Example: Birth certificate or legal adoption certificate.

a) Statutory declaration 1 (SD1) form completed and signed by the new certificate owner which is the person covered. The SD1 Form must be stamped with RM10 stamp duty and signed and witness in front of the Commissioner of Oath.

b) Company official letter informing on the released of the ownership of the certificate to the person covered. Letter must be written on the company’s official letter head and signed by company’s authorized person and stamped with the company’s stamp.

c) Clear copy of the new certificate owner’s NRIC/biometric copy or a clear copy of valid passport is needed.

d) Tax Residency Self-Certification form (Individual) of the person covered/new certificate owner.
